This form is designed so that Members can request assistance if they are experiencing difficulty, so that we can work with them remotely to consider temporary, flexible repayment arrangements.

Following submission of this form a member of staff will be in contact by phone to discuss your personal circumstances, we will have facilities so that you can submit any documentation we may require to consider your application for a flexible arrangement.

I hereby apply to the Credit Union for a Temporary Arrangement

I acknowledge that the Credit Union may require and request supporting documentation to fully consider my application, for example a Letter from my Employer or Proof of Self-Employment.

I understand and acknowledge that the Arrangement is a temporary arrangement which, if approved, commences and ends at the dates that will be specified in writing by the credit union and this temporary arrangement will be facilitated by the credit union for the reason that will be specified in writing

I further understand and acknowledge that the terms of my original Credit Agreement will may be amended temporarily for the time period that will be specified in writing and will return to my original Credit Agreement on the date this temporary arrangement ends.

I agree to the terms and conditions for access to and use of the Credit Union website and web forms and to the Processing of my data, which may include sensitive data, for the purposes of reviewing this application in accordance with the Privacy Notice
